Important Forms

1. Medical, Emergency Contact, and Pick-Up Authorization

Starting with the 21-22 school year, students' medical information and emergency contacts will be securely stored in Blackbaud, our Student Information System. We have done our best to import the medical information and emergency contacts we have for returning families. The one exception is information about medications, which could not be imported.

ALL RETURNING FAMILIES must log into the Parent Portal (use the "Login" link on www.spes.org), go to their child's profile (on a desktop, click "Children" and your child's name in the upper left hand corner. When your child's profile appears, click "Medical" and review the information that appears there. If there are medications you want the school to know about, you will need to add them. Then click "Contact Card" and review the emergency contacts. You can add, delete, and edit directly. You will also need to click the link for school forms to add some additional information about over the counter medications and more.

ALL NEW FAMILIES must log into the Parent Portal (use the "Login" link on www.spes.org) and click the link that will appear at the top to take you to forms. You will be prompted to complete forms with your child's medical information, emergency contacts, and more. 

This information needs to be submitted by August 13, 2021. If you have questions, please contact Director of Admissions & Enrollment Nicole Start (nstart@spes.org).

2. Immunization and Health Exam

Students attending St. Paul’s School must have documentation on file of all immunizations received. In addition, first graders must submit evidence of a health exam and seventh graders must submit documentation that they have received the required Tdap (whooping cough) booster. Information about the all-school immunization requirement is here (including information about the seventh grade booster.) First grade health exam information is here. You can download the first grade health exam form directly here.

3. Register for After School Program

We will be able to host our After School Program this fall! Please check our After School Program page for more information on how to register.

5. Live Scan Fingerprinting Information

In order to ensure the safety of our students, we have implemented a fingerprinting policy for adult chaperones. All parents who wish to chaperone field trips or accompany students for swimming at the YMCA must be fingerprinted beforehand. 
 
Please click here to download a Live Scan Service Form and ensure the St. Paul's unique ID and Mail Codes are included: A6466 (ORI), 07275 (Mail). Here is a list of places that offer Live Scan fingerprinting. You can be reimbursed for the cost of the fingerprinting by submitting your receipt and a reimbursement form to the School.

You only need to submit fingerprints once during your time at St. Paul's; if you submitted them in a previous year, you do not need to do it again. If you have had fingerprints done for another organization (not St. Paul's), you still must have them done again and designate St. Paul's as an organization that has permission to receive the results.  If you have any questions, please contact Associate Business Manager Tutu Tequame (etequame@spes.org).

6. Allergy Action Plan Form

If your child has any type of suspected or active allergy reaction, please download and fill out the Allergy Form.

Click here for a version you can fill out online and then print (or print and then complete by hand). Please submit it to Heather Debose, Lower School Secretary (K-5) or Laura Lance, Middle School Coordinator and School Registrar (6-8).

7. Volunteer Interest Survey

We are excited to announce know that many wonderful events that rely on parent volunteers like our beloved Book Fair, OakTown, assisting with admissions, supporting development, and other events are scheduled to happen the '21-'22 school year! 

Please complete this very brief survey to let us know what types of volunteering you may be interested in and/or what skills and talents you have that you would be willing to share. Click here to complete the survey.
Other Important Information

Update Your Address, Phone Number, and Email

  • Log into the School Portal, click on the down arrow next to your name at the top right corner, select Profile, and use the pencil icon to edit fields. You may also update information for a partner or spouse. The School uses your home address and email for the online directory and School Directory. Please update this information, as needed, throughout the year.
  • New Users: If you're new to St. Paul's School Portal, you'll need to click on the "First Time Login" link underneath the username and password boxes before you can update your information. 

School Foodies

St. Paul's will continue to work with our lunch provider, School Foodies, five days a week for the 21-22 school year. School Foodies gives parents the option to order lunch to be delivered for their child.

Pizza Day Info

We have made the decision not to have Pizza Day for at least the start of the 2021-2022 school year. School Foodies lunches will be available five days a week for parents who would like to order lunch. (See flyer above.)

Sync the St. Paul's Calendar to Your Calendar

Q.  How do I sync the St. Paul's calendar to my personal calendar?
A.  Log in. Go to Calendar. Then click on the icon in the right top corner next to the print button. This is the icon for the iCal RSS feed. Follow the instructions in the pop up window.

On your personal calendar, find the "subscribe" button. Enter the url into the subscribe feature and click add.

The St. Paul's calendar should now appear on your calendar.
