Important Forms

1. Medical, Emergency Contact, and Pick-Up Authorization

RETURNING FAMILIES: You are required to complete a form to inform the school which over the counter medications we're permitted to dispense and who has permission to pick your child up from school. (This information will not carry over from last school year.) Please log into the Parent Portal and you will see an alert near the top of the screen that you have a form to complete. This is due by August 12.

Your child's medical and emergency contact information will remain the same as last school year unless you make changes. 

You can view and edit emergency contacts and medical information  by logging into the Parent Portal and going to your child's profile (on a desktop, click "Children" and your child's name in the upper left hand corner.) 

Click "Medical" to view medical information. Click "Contact Card" to view emergency contacts. You can add, delete, and edit directly on the site. (Watch video here for step-by-step directions.)

ALL NEW FAMILIES must log into the Parent Portal. (Watch this first time login video to see how to access the Parent Portal.) Click the link that will appear at the top to take you to forms. You will be prompted to complete forms with your child's medical information, emergency contacts, and more (watch video here). 

This information needs to be submitted by August 12, 2022. If you have questions, please contact the Admissions & Enrollment Office (admissions@spes.org).

2a. COVID Vaccine AND Immunization Documentation

St. Paul's students are required to be vaccinated against COVID or to provide documentation of an exemption from the vaccine. All students must also submit a current immunization record on file at the School per the California Department of Public Health.

Please use this form to submit your child's complete and up to date immunization record, including the section showing the COVID vaccine (or submit documentation of your child's exemption.) This information is due by Friday, August 12, 2022.

If you have multiple children attending St. Paul's, please fill out this form separately for each child.

2b. 1st Grade Health Exam Requirements

In addition to an up to date immunization record, first graders must submit evidence of having a health exam with their pediatrician within the last 18 months. First grade health exam information is here. You can download the first grade health exam form directly here.

3. Register for After School Program

Registration is now open for ASP for 2022-2023! Please check our After School Program page for more information on how to register.

5. Live Scan Fingerprinting Information

In order to ensure the safety of our students, we have implemented a fingerprinting policy for adult chaperones. All parents who wish to chaperone field trips or accompany students for swimming at the YMCA must be fingerprinted beforehand. 
 
Please click here to download a Live Scan Service Form and ensure the St. Paul's unique ID and Mail Codes are included: A6466 (ORI), 07275 (Mail). Here is a list of places that offer Live Scan fingerprinting. You can be reimbursed for the cost of the fingerprinting by submitting your receipt and a reimbursement form to the School.

You only need to submit fingerprints once during your time at St. Paul's; if you submitted them in a previous year, you do not need to do it again. If you have had fingerprints done for another organization (not St. Paul's), you still must have them done again and designate St. Paul's as an organization that has permission to receive the results.  If you have any questions, please contact our Controller, Tutu Tequame (etequame@spes.org).

6. Allergy Action Plan Form

If your child has any type of suspected or active allergy reaction, please download and fill out the Allergy Form.

Click here for a version you can fill out online and then print (or print and then complete by hand). Please submit it to Heather Debose, Lower School Secretary (K-5) or Laura Lance, Middle School Coordinator and School Registrar (6-8).

7. Volunteer Interest Survey

Parent volunteerism is an essential and valued component of community life at St. Paul's. Parent volunteers support so many facets of the school, from helping in the library or classroom to supporting all-school events to offering more specialized expertise.

Your room parents will be in touch regularly throughout the school year with information about opportunities to volunteer. In the meantime, please complete this very brief survey to let us know what types of volunteering you may be interested in and/or what skills and talents you have that you would be willing to share. Other Important Information

Update Your Address, Phone Number, and Email

  • Log into the School Portal, click on the down arrow next to your name at the top right corner, select Profile, and use the pencil icon to edit fields. You may also update information for a partner or spouse. The School uses your home address and email for the online directory and School Directory. Please update this information, as needed, throughout the year.
  • New Users: If you're new to St. Paul's School Portal, you'll need to click on the "First Time Login" link underneath the username and password boxes before you can update your information. (Watch video here.)

School Foodies

St. Paul's will continue to work with our lunch provider, School Foodies, for the coming school year. School Foodies gives parents the option to order lunch to be delivered for their child.

You can read about how to view menus and place orders on this School Foodies flyer. (Note that you will not be able to place orders for the school year until August.)

Pizza Day Info

Pizza Day is a weekly tradition at St. Paul's! It helps raise money to cover the cost of our field trips. If your child is interested in having pizza for lunch each week, you can choose to order just for the first half of the year (with an option to order for the second half of the year in December) or you can order for the full school year all at once.

Sync the St. Paul's Calendar to Your Calendar

Q.  How do I sync the St. Paul's calendar to my personal calendar?
A.  Log in. Go to Calendar. Then click on the icon in the right top corner next to the print button. This is the icon for the iCal RSS feed. Follow the instructions in the pop up window.

On your personal calendar, find the "subscribe" button. Enter the url into the subscribe feature and click add.

The St. Paul's calendar should now appear on your calendar.
