Important Forms

1. Medical, Emergency Contact, and Pick-Up Authorization

All new and returning families will receive an email in mid-July with a link to complete your child's medical and emergency information. This form needs to be completed and submitted by August 12, 2020.

2. Immunization and Health Exam

Students attending St. Paul’s School must have documentation on file of all immunizations received. In addition, first graders must submit evidence of a health exam and seventh graders must submit documentation that they have received the required Tdap (whooping cough) booster. Information about the all-school immunization requirement is here (including information about the seventh grade booster.) First grade health exam information is here. You can download the first grade health exam form directly here.

3. Register for After School Program

We are reviewing whether it will be possible to have our After School Program this fall. We will post additional information here as soon as it is available.

5. Live Scan Fingerprinting Information

In order to ensure the safety of our students, we have implemented a fingerprinting policy for adult chaperones. All parents who wish to chaperone field trips or accompany students for swimming at the YMCA must be fingerprinted beforehand. Although we don't anticipate that we will be able to have field trips or swimming this fall, we are keeping this form available in the event that we are able to start such trips later in the year.
 
Please click here to download a Live Scan Service Form and ensure the St. Paul's unique ID and Mail Codes are included: A6466 (ORI), 07275 (Mail). Here is a list of places that offer Live Scan fingerprinting. You can be reimbursed for the cost of the fingerprinting by submitting your receipt and a reimbursement form to the School.

You only need to submit fingerprints once during your time at St. Paul's; if you submitted them in a previous year, you do not need to do it again. If you have had fingerprints done for another organization (not St. Paul's), you still must have them done again and designate St. Paul's as an organization that has permission to receive the results.  If you have any questions, please contact Associate Business Manager Tutu Tequame (etequame@spes.org).

6. Allergy Action Plan Form

If your child has any type of suspected or active allergy reaction, please download and fill out the Allergy Form.

Click here for a version you can fill out online and then print (or print and then complete by hand). Please submit it to Heather Debose, Lower School Secretary (K-5) or Laura Lance, Middle School Secretary (6-8).

7. Volunteer Interest Survey

We want to connect parents with volunteer opportunities that are interesting to them! Please complete this very brief survey to let us know what types of volunteering you are interested in and/or what skills and talents you have that you would be willing to share. Click here to complete the survey.
Other Important Information

Update Your Address, Phone Number, and Email

  • Log into the School Portal, click on the down arrow next to your name at the top right corner, select Profile, and use the pencil icon to edit fields. You may also update information for a partner or spouse. The School uses your home address and email for the online directory and School Directory. Please update this information, as needed, throughout the year.
  • New Users: If you're new to St. Paul's School Portal, you'll need to click on the "First Time Login" link underneath the username and password boxes before you can update your information. 

Earthquake Supplies

Every student must have an earthquake kit stored on campus. Please provide the items on this Earthquake Supply List and ensure your child has the kit in their classroom on the first day of school.




School Foodies

Keeping our student and faculty health and safety concerns at the forefront, St. Paul’s is evaluating if and how we can continue to work with our lunch provider, School Foodies, for the 20-21 school year.

Sync the St. Paul's Calendar to Your Calendar

Q.  How do I sync the St. Paul's calendar to my personal calendar?
A.  Log in. Go to Calendar. Then click on the icon in the right top corner next to the print button. This is the icon for the iCal RSS feed. Follow the instructions in the pop up window.

On your personal calendar, find the "subscribe" button. Enter the url into the subscribe feature and click add.

The St. Paul's calendar should now appear on your calendar.

Photo and Video Release

When you enrolled your child, you either agreed or disagreed to the following:

I/We hereby authorize and give full consent to St. Paul's School and Summer Camp to take and publish photos, videos, and the like in which my child appears while he or she is participating in school- and camp-related classes, activities, and events throughout the time that he/she is enrolled at St. Paul's School or Summer Camp. 

I/We further agree that St. Paul's may use these photographs or videos in materials that promote the School such as the website, brochures, magazines, posters, advertisements, and social media.

I/We will alert St. Paul's in writing if any changes in this permission are to be made.
